Amaryl MV (glimepiride, metformin, voglibose) is a type 2 diabetes treatment containing three active ingredients in one tablet, ideal for patients for whom one or two medications do not produce sufficient results.

The three active ingredients in Amaryl MV are:

• Glimepiride promotes insulin production by the pancreas, lowering blood sugar as the new, additional insulin converts it to energy or stores it in tissues.
• Metformin lowers blood sugar by reducing its production in the liver, inhibiting glucose absorption, and increasing glucose uptake by muscle and fat.
• Voglibose inhibits the digestion of carbohydrates and delays the absorption of glucose, preventing post-meal surges of glucose.

Most patients taking three antidiabetic treatments have already tried one or two, and found them to be insufficient. It is rare that patients are started on three medications at first, though it does sometimes happen.

Patients taking all three of the same medications found in Amaryl MV, but rather than taking each individually are switching to Amaryl MV, should try to match the doses as much as possible. All other patients---those new to one or more of the medications in Amaryl MV---are typically started on low doses unless explicitly instructed otherwise by a doctor.

In other words, most patients:

• Start with low doses of 1 mg/500 mg/0.2 mg glimepiride/metformin/voglibose.
• Take this dose for one or two weeks; it takes this long for maximum benefit to be seen.
• After one or two weeks, dosage may be increased, if needed, in small increments every two weeks.
• Blood sugar should be monitored closely at the start of treatment and following any dosage adjustments.

Note that taking multiple Amaryl MV tablets provides the same benefit as taking a single higher-dose tablet; for example, two 1 mg/500 mg/0.2 mg tablets is the same as taking a single 2 mg/1000 mg/0.4 mg tablet. It may not be advisable to increase dosage by so much, though; doctor involvement is important in determining dosage levels. Doctor instructions are individualized for each patient and should be followed.

Lifestyle Changes

Patients given a prescription to buy Amaryl MV will almost certainly be advised to make lifestyle changes, such as:

• Reach and maintain a healthy weight
• Get regular exercise
• Control blood sugar
• Control blood cholesterol

Potential Complications

The three active ingredients in Amaryl MV put patients at an increased risk of hypoglycemia, or low blood sugar. Close monitoring of blood sugar often prevents serious issues, but they can happen even to the most attentive patient and may require emergency medical attention. All patients who buy Amaryl MV should be aware of symptoms and how to manage them. The prescribing doctor should make this information available, and there is also an abundance of information online.

Most side effects are mild and transient, meaning they typically stop occurring in the first few weeks of treatment. Rarely more serious side effects develop, which warrant attention by a doctor, including:

• Severe gastrointestinal pain
• Severe itching
• Chest pain
• Hypoglycemia

Other side effects may develop which are not detectable by the patient. These include:

• Low red or white blood cell counts
• Low blood platelets
• Altered kidney or liver enzymes
• Low sodium
• Lowered immunity
• Impaired bile flow
